This release is for the use of all students, and their caretakers of Suzuki Talent Education of Waterloo (STEW). It will remain in effect until retracted by the participant.
In the course of activities, representatives of STEW, participants and staff may take photographs or otherwise record events. These photographs and recordings may be used for educational purposes, teacher development purposes, and or to promote Suzuki Talent Education of Waterloo. We will not use names of participants for any advertising materials. Please advise us if you are willing to have your own image and voice and/or your child’s image and voice/instrument used for promotional or educational purposes by Suzuki Talent Education of Waterloo as indicated below:
I give permission to STEW to photograph and record me or my child on still photographs, motion picture film, audio tape, video tape or digital media and to use this material, and/or similar material provided to (STEW) by me, in whole or in part, now and in the future, through the media of television, film, internet, multi-media presentation, radio, audiotape, videotape, in printed form and display form for teacher development or the promotion of STEW. I assign and transfer to Suzuki Talent Education of Waterloo any and all proprietary rights, including copyright, and waive all personality rights, which I may have or my child may have in this material.
STEW is only responsible for official uses of photographs and recordings. Any personal uses by other participants and volunteers outside of the promotional uses outlined above are not monitored by or the responsibility of STEW
